Summer Registration form

Step 1:

Download or print our Registration Form and fill it out in its entirety. Be sure to select the right registration form for the session you want to enroll in, either Fall through Spring, Thanksgiving Camp, or Summer.

Step 2:

Return your completed Registration form. Email your form to studio@cdt.dance, mail, or drop off your form in person at CDT.

Step 3:

Finally, pay the amount due to reserve your spot. Please see our Payments Page for all our payment options.

You will receive a confirmation of your enrollment via email!
